Application for School Meals

Park City Prep participates in the CEP (Community Eligibility Provision) Program for school breakfasts and lunches. This program allows ALL students to receive school breakfast and lunch at no charge; however, parents must complete this form:

1. Opt-In/Opt-Out School Meals Form

The form can be found in the drop-down menu of the Parents section on the website.

The form should be returned to your children's homeroom teachers, who will deliver them to the Operations Manager, David Figueroa. Forms is due no later than Friday, September 3, 2021.
